| The forms below are instrumental in processing your loan. We will walk you through the process, and discuss each form with you. Please read all of the following carefully. |
| You will need Adobe Acrobat to view and print these forms. It is likely you have the software already. If not, you can download it HERE free. |
| After we discuss your loan project I may request that you print these forms, fill them out and fax them to me. |
| For the forms designated “Legal format” please do the following. |
- Insert 8.5”x14” LEGAL paper into your printer
- Left click the link and the form will appear after it loads on your screen.
- Click “File” then click “Print...”
- Make sure the correct printer is selected. Click “Properties”
- Set your Paper to 8.5”x14” LEGAL
- Hit “OK” to make settings and “OK” again to print.
- Depending on your printer you may have to change your settings back to 8.5”x11” LETTER paper when you switch back to printing on that size paper.

| The above are general instructions. Every printer and operating system is a little different. If you try to print them on regular 8.5”x11” LETTER paper, the forms may print at a reduced scale, which can not be processed. |

| In addition, the following will be necessary to process your loan. |
- If you are salaried, provide two years' W-2 and one month of pay stubs. If you are self-employed, provide two years' tax returns and a YTD profit and loss statement.
- If you own rental property, please provide rental agreements and two years' tax returns.
- If you wish to speed up the approval process, please also provide three months' bank statements for each bank, stock and mutual fund account.
- Provide recent copies of any stock brokerage or IRA/401K accounts that you may have.
- If you are requesting a cash out refinance please provide a letter explaining what you plan to do with the proceeds.
- Provide a copy of divorce decree, if applicable.
- If you are NOT a US citizen, provide a copy of your green card (front & back), or if you are NOT a permanent resident provide your H-1 or L-1 visa.
